Mong B: no, your size should be the same Candace C: To make sure I understand your question, do you mean, "as the heel gets taller, should you get a LARGER size, as compared to a flat" If this is what you meant, then NO. I get a SMALLER size than what I wear in flats. I wear a US 8 or 8.5 in flats, and I wear a US 7 or 7.5 in heels. The reason I do this, is because as you walk in the heel and it begins to break in, your foot will move forward in the foot bed about a quarter inch. So to keep the heel fitting properly and to keep the support for your ankle in place, this is why I go DOWN a size or half-size. When you first try on the heel, it WILL be tight in the toe and probably a little uncomfortable, if you are not use it. However, your toes SHOULD NOT be trying to cross over each other. If this is the case, adjust the size. If your toes still want to cross each other, you have the wrong type of heel for your feet. Sorry.
